What Services are Available?

Who is Eligible?
Children birth to age 20 receiving Medicaid
Patients over the age of 20 receiving Medicaid may qualify for emergency dental care.
Emergency dental care is only provided during normal hours of operation.

When?
The Dental Clinic is open from
8:00 a.m. to 5:00 p.m., Monday through Thursday
and
8:00 a.m. to 12:00 p.m., Friday

How do I get an appointment?
Call (850) 482-1240.

You must bring the following to your child’s appointment:
your child’s Social Security Card and Medicaid Card;
your Driver’s License or other form of photo ID.
If you are not the parent, you will need to bring proof Guardianship or custody.

Broken Appointment Policy:
When appointments are made they are for your convenience and we expect them to be kept. If you fail to keep
an appointment or show up late, you are depriving someone else of this time. It is your responsibility to keep
track of your child’s appointment. If you are unable to keep an appointment, please notify us at least 24 hours
in advance. Failure to do so may result in suspension or dismissal from the dental program.

                    Mobile Dental Program

Jackson County Health Department Dental Program also provides a school based dental care program, Smiles On Wheels. The mobile unit visits elementary schools in Jackson, Calhoun, Holmes, Liberty, and Washington Counties providing services to Pre-K through 3rd Grade students who are eligible for Medicaid or the Free Lunch Program. The mobile unit is staffed by a licensed dentist, two dental hygienists, and one dental assistant.
