Primary Care Services

Kongsak Chantornsaeng (Dr. Chan)
Jackson County
Health Department coordinates with the Florida Department of Health
in promoting and protecting the health and safety of the people in Jackson
County.
We strive to prevent and control the spread of disease and provide basic
personal health care services to individuals unable to obtain them elsewhere.
Clinic services are provided by appointment Monday through Thursday from
7:30 a.m. until 4:00 p.m. and Friday 7:30 a.m. until 12:00 p.m. at the
Marianna Clinic; and every 2nd and 4th Tuesday 9:00 a.m. until 3:30 p.m.
at the Graceville Clinic.
Clinic Services Provided at Jackson County Health Department include:
l Comprehensive
Adult and Child Health Medical Services Including Screenings, Health Education,
Evaluation, Treatment, Referral and Follow-up Services
l Primary
Care Services to individuals below 100% Poverty Level. Medical treatment
services are provided to eligible children and adults by a Physician or
ARNP (Advanced Registered Nurse Practitioner). Certification of
financial eligibility is required. Services include evaluation and
treatment for acute and chronic illness, counseling, health teaching,
referrals, and follow-up. Medications are available for eligible
patients.
